Symptoms

Loud snoring
Witnessed apneas
Daytime sleepiness
Morning headaches
Difficulty concentrating
Irritability
Nocturia
Decreased libido
Dry mouth
Gasping/choking at night

Treatment Options

CPAP therapy
BiPAP
Auto-titrating PAP
Oral appliance therapy
Hypoglossal nerve stimulation
Positional therapy
Weight loss
Upper airway surgery
Bariatric surgery

Risk Factors

1Obesity (BMI >30)
2Male gender
3Age >40
4Large neck circumference
5Craniofacial abnormalities
6Family history
7Alcohol/sedative use
8Smoking
9Nasal congestion
10Hypothyroidism
11Acromegaly

Diagnostic Methods

  • 1Polysomnography (gold standard)
  • 2Home sleep apnea test
  • 3Epworth Sleepiness Scale
  • 4Physical examination (Mallampati score, neck circumference)
  • 5Drug-induced sleep endoscopy
  • 6Craniofacial imaging

Prognosis

Excellent with treatment. CPAP reduces cardiovascular events and mortality. Untreated OSA associated with hypertension, heart disease, stroke, diabetes, and increased mortality. Motor vehicle accident risk increased.

Prevention

  • Weight management
  • Avoid alcohol before bed
  • Avoid sedatives
  • Sleep position modification
  • Treat nasal congestion
  • Smoking cessation
  • Regular exercise

Research Status

Continuous positive airway pressure (CPAP) remains gold standard. Mandibular advancement devices effective for mild-moderate. Inspire (hypoglossal nerve stimulation) for CPAP-intolerant. Positional therapy. Weight loss effective. Pharmacotherapy limited.
