Prognosis

Despite trials of 2+ appropriate antiepileptic drugs, 30-40% of patients remain refractory. Epilepsy surgery offers 60-80% seizure freedom in carefully selected candidates with focal epilepsy. Vagus nerve stimulation provides 50%+ seizure reduction in 40-50% of patients. Ketogenic diet helps 30-50% of children and some adults. Sudden unexpected death in epilepsy (SUDEP) risk is 1 in 1,000 annually. Early surgical referral improves outcomes; delay reduces success rates.
